To Book An Appointment...

Once you have reviewed the website and decided that you would like to book a consultation with Dr Flamer, please follow these next steps:

1.Call our office to schedule an appointment

If you prefer to "jump start" your program, you may request to have your testing done prior to your first visit and proceed directly to the 45 min second visit. Please let us know and we can arrange to have your blood test requisitions and saliva test kits sent to you. Your first visit with Dr Flamer will be your 45 min consultation.

2. Download our intake forms from the page (see below). These will need to be filled out in advance and brought to your first visit.

3. If you have had any recent pelvic ultrasounds or mammograms done, please request a copy from your family physician and bring it to your visit (or request them to fax it to our office). This is not mandatory for your visit with Dr Flamer however if it has been it will be helpful for her to have.

Please be aware of our cancellation policy, as listed on the "Our BHRT Program page". We require 24 hours cancellation or rescheduling notice or you will be charged for your visit.
